Acupuncture for PMDD
PMDD isn't simply a more intense form of PMS. It's a distinct diagnosis affecting an estimated 3-8% of menstruating women, involving a heightened sensitivity to normal hormonal fluctuation, specifically progesterone's metabolite allopregnanolone, in the luteal phase. Hormone levels are typically normal on a blood test. The response to them isn't. That distinction matters, because it's the reason "your bloods are normal" doesn't mean nothing is happening.
The biomedical picture
PMDD is understood as an abnormal central nervous system sensitivity to normal luteal phase hormonal shifts, affecting GABA signalling via allopregnanolone. Symptoms, mood changes, irritability, anxiety, physical symptoms, are confined to the luteal phase and resolve with menstruation, which is the key diagnostic marker separating it from generalised mood or anxiety disorders that simply happen to worsen premenstrually.
Where acupuncture fits
Acupuncture has evidence for modulating the HPA axis stress response and supporting nervous system regulation, relevant given PMDD's root in CNS sensitivity rather than hormone level abnormality. It also supports general luteal phase progesterone patterns and reduces the physical symptom load (breast tenderness, bloating, cramping) that often compounds the mood symptoms.
In TCM, PMDD's cyclical, luteal-phase-specific nature points most often to Liver Qi stagnation (irritability, tension, mood symptoms that build and then release with bleeding), frequently alongside Kidney or Spleen deficiency underlying the pattern. Treatment concentrates in the luteal phase specifically, working with your cycle's biology rather than treating identically every week regardless of phase.
What treatment looks like
Weekly or twice-weekly sessions concentrated from ovulation through to menstruation, alongside practical luteal phase support around sleep, blood sugar and nervous system regulation, since all three influence symptom severity.
PMDD FAQ
How is PMDD different from PMS?
PMDD involves significant mood symptoms, such as depression, anxiety, irritability or hopelessness, severe enough to disrupt daily functioning, confined specifically to the luteal phase. PMS involves milder physical and emotional symptoms that don't typically reach that level of disruption.
Will acupuncture show up as changing my hormone levels for PMDD?
Not necessarily. PMDD is generally driven by CNS sensitivity to normal hormone fluctuation rather than abnormal hormone levels, so acupuncture's relevance lies more in nervous system regulation than in "correcting" hormones that likely aren't the problem.
When in my cycle should I have acupuncture for PMDD?
Treatment is usually concentrated in the luteal phase, from around ovulation to the start of your period, since that's when the CNS sensitivity driving PMDD symptoms is most active.
